Where a document starts
Each task starts from a standard assessment of its hazards, taken from a library grounded in 52 HSE publications and approved codes of practice. That part is the same on every job.
The requirements from your project are then worked through it, clause by clause. Where a requirement changes a clause, the clause is rewritten and the requirement is recorded as the reason. Where no requirement applies, the standard text stays as it is.
Every change is marked
Every line the project changed is highlighted, and opens to the requirement behind it and the page that requirement came from. Standard text is left unmarked.
Whoever approves the document can see exactly which parts are specific to this site, and why.
4.2 Excavation and support
The connection trench is dug by 3t excavator to invert, 1.4 m below existing surface, working back from the manhole toward plot 18.
Sides are supported by proprietary trench box from 1.2 m, set from outside the excavation. No operative enters an unsupported trench at any depth.

Gaps are left as questions
Some things aren't in the pack: who countersigns the permit to dig, which temporary works coordinator is appointed, the route to the nearest A&E. Ramsgen doesn't guess them. Each one is left as a question in the document, so you can see what still needs answering before it goes out.
Yours to edit
You can edit any line of the document. Ramsgen doesn't sign it or decide that it's adequate; the competent person who puts their name to it is you. When you want a finished RAMS checked against the project, that is a review.
